ThemonoclonalantibodyRCR-252recognizeshumanendothelialproteinCreceptor(EPCR),ahighlyglycosylatedtypeItransmembraneproteinof221-amino-acids.Theseaminoacidscompriseanextracellulardomain,a25-aatransmembranedomain,andashort(3aa)intracytoplasmicsequencecodingforan~46kDaprotein.Deglycosylationwillreducetheproteinmassto25kDa.EPCRisexpressedstronglyontheendothelialcellsofarteriesandveinsinheartandlung,lessintenselyincapillariesinthelungandskin,andnotatallintheendotheliumofsmallvesselsoftheliverandkidney.
EPCRisthereceptorforproteinC,akeyplayerintheanticoagulationpathway.TheproteinCanticoagulantpathwayservesasamajorsystemforcontrolingthrombosis,limitinginflammatoryresponses,andpotentiallydecreasingendothelialcellapoptosisinresponsetoinflammatorycytokinesandischemia.Theessentialcomponentsofthepathwayincludethrombin,thrombomodulin,theendothelialcellproteinCreceptor(EPCR),proteinCandproteinS.Thepathwayisinitiatedwhenthrombinbindstothrombomodulinonthesurfaceofendothelium.EPCRaugmentsproteinCactivationbybindingproteinCandpresentingittothethrombin-thrombomodulinactivationcomplex.ActivatedproteinC(aPC)retainsitsABIlitytobindEPCR,andthiscomplexappearstobeinvolvedinsomeofthecellularsignalingmechanismsthatdown-regulateinflammatorycytokineformation(TNF,IL-6).EPCRisshedfromthevasculaturebyinflammatorymediatorsandthrombin.EPCRbindstoactivatedneutrophilsinaprocessthatinvolvesproteinase3andMac-1.FurThermore,EPCRcanundergotranslocationfromtheplasmamembranetothenucleus.
EPCRcanbecleavedtoreleaseasolubleform(sEPCR)inthecirculation.ThissEPCRisdetectedasasinglespeciesof43kDa,resultingfromsheddingofmembraneEPCRbytheactionofametalloprotease,-whichisstimulatedbythrombinandbysomeinflammatorymediators.SolubleEPCRbindsPCandaPCwithsimilaraffinity,butitsbindingtoaPCinhibitstheanticoagulantactivityofaPCbyblockingitsbindingtophospholipidsandbyabrogatingitsabilitytoinactivatefactorVa.sEPCR-canbedetectedinplasma.Innormalpersons,sEPCRispresentinlevelsof83.6+/-17.2ng/ml.ElevatedlevelsofsEPCRarepositivelycorrelatedtoahigherriskforthrombosis.Furthermore,ahaplotype(A3allele)hasbeenlinkedtoelevatedlevelsofsEPCR(264+/-174ng/ml).
