β-lactamsinterferewithPBP(penicillinbindingprotein)activityinvolvedinthefinalphaseofpeptidoglycansynthesis.PBP’sareenzymeswhichcatalyzeapentaglycinecrosslinkbetweenalanineandlysineresiduesprovidingadditionalstrengthtothecellwall.Withoutapentaglycinecrosslink,theintegrityofthecellwallisseverelycompromisedandultimatelyleadstocelllysisanddeath.Resistancetoβ-lactamsiscommonlyduetocellscontainingplasmidencodedβ-lactamases;however,carbapenems,includingdoripenem,arehighlyresistanttoβ-lactamases.
