Fibrinogen(FBG)isahomodimerofmolecularmass340kDa,madeupoftwosetsofα,β,γpolypeptidechains,andsynthesizedintheparenchymalcellofthehepatocyteandinthemegakaryocyte.FBGplaysamajorroleincoagulation,andbothelevatedanddecreasedlevelshaveclinicalsignificance.Uponcleavagebythrombinintheinitialstagesofcoagulationactivation,FBGself-assemblestoyieldafibrinclotmatrixthatsubsequentlyiscrosslinkedbyFactorXIIIatoformaninsolublenetwork.FBGalsobindstotheplateletglycoproteinIIb/IIIareceptorsoastoformbridgesbetweenplatelets,thusfacilitatingaggregation.ElevatedplasmaFBGhasbeenidentifiedasanindependentriskfactorforcoronaryatherosclerosisandischemicheartdisease.IndividualswithcongenitalabsenceofFBG,termedafibrinogenemia,haveprolongedbleedingtimes.
