IntendedUse	TheHSV2IgGrecombinantELISAallowsthedetectionofHSV2infectioninpresenceofantibodiestoHSV1inhumanserumorplasma(citrate).
Introduction	HerpessimplexisanenvelopedDNAvirus(150-200nmindiameter)belongingtothealpha-herpesviridae.Basedonantigenic,biochemicalandBIOLOGicallydifferencesitcanbedividedintotwoSEROtypes,HSV-1andHSV-2.Manistheonlyknownnaturalhostandsourceofthevirus.HSVtype1typicallycausesoralherpes,whileHSVtype2typicallyaffectsthegenitalarea.Mostofthetime,HSV-1andHSV-2areinactive,or“silent”,andcausenosymptoms,butsomeinfectedpeoplehave“outbreaks”ofblistersandulcers.OnceinfectedwithHSV,peopleremaininfectedforlife.Herpessimplexvirusesareamongstthemostcommoninfectiousagentsofman,andeitherHSVtypeappearstobecapableofinfectingsimilarbodysites.Ahighpercentageoftheadultpopulation isseropositive(appr.90%HSV-1,independenceonthesocio-economicstatus10-30%HSV-2).PrimaryHSV-1infectionusuallyoccursinearlychildhood(6to18monthsofage).HSV-2usuallyproducesmildsymptoms,andmostpeoplehavenorecognizedsymptoms.PersonsatriskarechildrenwithinheritedT-celldeficienciesandpatientswhoareimmunosuppressedbecauseofinfection(e.g.HIV),transplantation,orcancertherapy. HSV-2cancausepotentiallyfatalinfectionsininfantsifthemotherissheddingvirusatthetimeofdelivery.-HSV-2mayplayamajorroleintheheterosexualspreadofHIV:herpescanmakepeoplemoresusceptIBLetoHIVinfection,andHIV-infectedindividualsmoreinfectious. Thepresenceofvirusresp.infectionmaybeidentifiedby Microscopy:CPE,IF PCR Serology:DetectionofantibodiesbyELISA.
Principlesoftheassay	RecombinantgG2proteinofHSV-2showdiffusebandinSDSelectrophoresiscorrespondingtogG2whichisfoundatapproximately43kD.ThisantigenwillallowthedetectionofHSV2infectioninthepresenceofantibodiestoHSV1.Thisisimportantasthe%ofthepopulationwithHSV1increasesthroughoutlifeupto>90%atoldage.HSV2isgenerallysexuallytransmittedandinallsocietiesisassociatedwiththeamountofsexualactivityandnumberofpartners.Genitalherpesisimportanttodiagnoseinpregnancyassheedingviruscanleadtocongenitalinfectionofthechildwitchisinvariablyserious.ThequalitativeimmunoenzymaticdeterminationofIgG-classantibodiesagainstHSVType2isbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique. MicrotiterstripwellsareprecoatedwithHSVType2recombinantantigenstobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledanti-humanIgGconjugateisadded.ThisconjugatebindstothecapturedHSVType2specificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofHSVType2specificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.
StorageandStABIlity	Thereagentsarestableuptotheexpirydatestatedonthelabelwhenstoredat2..8°C.
LimitationsoftheTest	Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasserologicaldata.Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue.
