ThehumanCOMPmoleculeisapentamericproteinoffiveidenticaldisulfide-linkedsubunitswithatotalmolecularmassof434kDa.COMPwasfirstdescribedincartilagebuthasrecentlyalsobeenfoundintendonsandsynovialmembranes.IthasnotbeenpossIBLetodetectCOMPinculturemediumfromotherconnectivetissuessuchasskinorlung.
Whenarticularcartilagematrixisdegradedbyadiseaseprocess,proteinfragmentsareproducedthatdif-fuseoutintothejointfluid.Someoftheseproteins,suchasCOMP,subsequentlyappearinthebloodstreamandcanbeusedtomonitortheprogressofthecartilagedegradationininflammatoryjointdiseasessuchasrheumatoidarthritisandosteoarthritis.AquantitativerelationhasbeenshownbetweentheconcentrationsofCOMPinserumandcartilagedegradationusingrADIographicchangesasasurrogateclinicalendpoint.ThishasbeenfurtherprovedinexperimentallyinducedarthritisinanimalmodelswheretheCOMPserumlevelwashighlycorrelatedtotheseverityofarthritisandalsotheclinicaljointscoreandhistopathologicalsignsofcartilageerosion.
